64-bit replay counters

[AH-HMAC-MD5] and [AH-HMAC-SHA1] state that the replay counter is 64
bits, but it points to the [ESP-DES-MD5] draft for an exmple of a 32-bit
replay counter source code.  Could it provide more information on how to
transform that 32-bit code to handle the newer 64-bit counters.
