How does an end-entity enroll to a CA ?   What protocol is used to
communicate the PKCS#10 certificate request to the CA ?

I  have read LDAPv2, in which I can ask for a particular certificate or a
CRL from a CA.  But it does not talk on how a end-entity will communicate a
certificate generation request to the CA. Am I missing something ?
