Thanks -- that's a useful summary.  I should point out that NIST has 
announced its Modes of Operation plans, at
http://csrc.nist.gov/encryption/tkmodes.html
The current plan is to specify the four current modes (ECB, CBC, CFB, 
and OFB), plus a counter mode, some time this spring.  I'd be loathe to 
see the IETF design its own counter mode in advance of the NIST 
standard.  They're also planning to specify other modes in the future.
