Readers, I have not done change bars on this one.

The document has been rototiled based upon suggestion to "make it more
RFC791-like". What was previously specified in terms of packet timing
diagrams is not specified as two intertwined state machine definitions
(forwarding and control plane). The previous packet timing diagram is now
just an illustrated example.  

I would strongly appreciate feedback on the readability and flow of this
document. I think that it is much improved in technical content.
