Look and Feel updates

PR#415996 relates to a suggestion to put rules for car pools on the distance editing page.

While seemingly a trivial edit, a way to do this without excessive clutter was not found.

PR#428730 points out that all the web forms listed the national organizer as a place to send comments. This information should have come out of the database.

These in general relate to PR#416010 asks for prettier pages in general.

The theme used for the main pages should be extended to the data entry pages. All pages were kept intentionally simply, using minimal server side scripting, no client side scripting and no dynamic generation of content. This was done to permit city coordinators to easily adapt the site to their own needs while not requiring excessive expertise.

A recommendation is to remove all the data entry pages from the per-city sites and maintain a more complicated server side configuration to permit a more sophisticated page design.

The downside of this is that these pages will only be partially customizable by city coordinators.

Of 15 cities running active campaigns, only one, Kamloops customized the data entry pages. This style of customizations (creation of a frame and use of city colour scheme) could easily be accommodated by a more sophisticated server side script on the data entry pages.
